I am in the care of a lactation specialist, and we are doing better.Do you know of any foods, herbs or medications that I should avoid because they may be decreasing my breast-milk supply?I would appreciate any information you have.Thanks.— S.G.DEAR S.G.: My sincere congratulations on the arrival of Nicole.Breastfeeding is as “natural” as it gets, but sometimes the process may not seem simple or natural at first. Some seek the guidance of a knowledgeable individual to smooth over the rough spots .and it is not that unusual for there to be at least a few rough spots. It is great that you are receiving advice, and I hope that things will improve as time passes. You use the term “lactation specialist.” I trust that you are referring to an appropriately trained individual, such as a lactation consultant certified by the International Board of Lactation Consultant Examiners (IBCLC credential) or a La Leche League Leader. In response to your question, there are many potential interactions between breast-feeding and foods, herbs and medications. That is why before using any herbs or medications while pregnant, actively trying to get pregnant, or as in your case, nursing, it is always best to play it safe and check with a health-care practitioner. In your particular case, there can be direct effects on your milk supply and your ability to nurse.Unfortunately, this is not a situation where I can give you a list of what to avoid. There are more things to consider, such as the possibility that the compounds you consume will find their way into the milk supply and into Nicole. Most foods, herbs and medications pass into the breast milk to some degree.Think of wholesome foods and essential nutrients, as they are what will allow your daughter to thrive. The role of the essential nutrients has stood the test of time, but there is less known about the potential longterm effects of herbs and medications.Because of this, a cautious approach is definitely called for.In some cases, a particular compound might be indicated, but this would be a decision based on your individual needs balanced with any potential risks. The decision needs to be made by you in concert with your lactation consultant and the other health professionals on your team.Best wishes to you and Nicole.DEAR DR.BLONZ: Recently, I have been seeing articles on the benefits of “folates.” These articles give the impression that while part of the B complex, it is a “new” component. Is folate the same as folic acid, or is it something different?I have looked at the two supplements I take, but neither show folate as a component of their multivitamin or B complex formulation.— THANKS DEAR THANKS: Throughout its history, the vitamin we call folic acid or folate has gone under many different names, including Wills Factor, anti-anemia factor PGA, vitamin M, vitamin Be, factor R, SLR factor, vitamin U, factor U, vitamin B9, vitamin B10 and vitamin Bll.Why so many names?Well, many different laboratories were doing research on the same substance at the same time, and many were working on related compounds that had the same vitamin activity.Folate, as it turns out, is a generic term that refers to a family of related compounds, the simplest of which is folic acid.We usually don’t find folic acid in nature, but rather they are folates, which you can think of as being a form of folic acid with different lengths of a particular side chain attached. In food, therefore, we hear of folates, while in supplements we tend to find folic acid.Folic acid appears to be a key compound in human nutrition. The increased frequency of articles on the benefits of folates reflects the fact that an inadequate intake of this nutrient is now being associated with a number of different ailments, including heart disease and certain birth defects. What’s of interest here is that you don’t need megadoses to avoid problems. You can get all the folate you need from a healthy diet.Good sources include green leafy vegetables, organ meats, legumes, orange juice, beets, avocado and broccoli.Ed Blonz, Ph.D., is a nutrition scientist and the author of “Power Nutrition”(Signet, 1998)and the “Your Personal Nutritionist” book series (Signet, 1996).¦ * ¦¦ » \ \ t — ¦— THE un — = Record____ CROSSWORDS Friday, September 10, 1999 page 23 ACROSS 1 Campus military grp.5 Steep slope 10 Eve’s second 14
